This option allows you to spend less time in our waiting room by doing the majority of your initial paperwork in advance. Please complete all forms in each section for each patient, either in the child (under 18 years old) or adult (18 and above years old) categories. Each form requires the patient's First Name, Last Name, and Date of Birth to be filled out. Some forms have other required answers that must be complete. Remember to click "Submit" when you're finished filling out information!
